A 57-year-old female is brought in by her relations to the casualty unit. She has no past medical history of note.

Her blood glucose level is 1.3 mmol per litre

She is given intravenous glucose and regains consciousness. Subsequently her blood is analysed by the biochemistry laboratory and her insulin level is found to be 56 pmol/L (<21pmol/L)
